Gabapentin is an anticonvulsant drug that is used to treat seizures caused by epilepsy and restless leg syndrome (RLS). In addition to partial seizures, it is also used to treat postherpetic neuralgia or pain that persists after an attack of shingles. Gabapentin is classified as a gamma-aminobutyric acid (GABA) analogue. BACKGROUND: Postherpetic neuralgia (PHN) is a common type of neuropathic pain occurring after resolution of herpes zoster rash. Although gabapentin is a widely used treatment, some disagreements exist about its efficacy and safety. Abstract Gabapentinoids (e.g., gabapentin and pregabalin) have been established as a treatment for postherpetic neuralgia (PHN), but their effects on the prevention of PHN are unclear. This systematic review aimed to evaluate the efficacy of gabapentinoids for acute herpes zoster (HZ) in preventing PHN. Keywords: Postherpetic neuralgia, Gabapentin, Herpes zoster, Prevention, Primary health care Background Herpes zoster (HZ), often called shingles, is a common disease characterized by a painful, unilateral vesicular eruption that is caused by reactivation of a dormant varicella zoster virus within the dorsal root or cranial nerve ganglia. Objective: This study aimed to systematically evaluate the clinical efficacy of gabapentin and pregabalin in the treatment of acute herpes zoster (HZ) neuralgia, including pain control and the occurrence of adverse effects. We performed a prospective randomized controlled study of 120 participants diagnosed with acute herpes zoster, aged 50 and over and complaining moderate to severe pain. All patients were treated with valacyclovir and acetaminophen. Gabapentin does not work to treat the virus-causing shingles, but rather it is used to address the pain from nerve damage that can occur in certain individuals at higher risk of developing complications from shingles, termed postherpetic neuralgia. Duration of Treatment: Gabapentin is taken for 7 to 14 days for shingles pain. Dosage Guidelines: Initial doses start low, increasing based on tolerance. Consult Healthcare Providers: Always discuss treatment options and adjustments. Pain Management: Gabapentin effectively reduces postherpetic neuralgia pain. Lifestyle Modifications:Lifestyle Background: Postherpetic neuralgia (PHN) is the most common complication of herpes zoster (HZ). Previous trials have reported that gabapentin can relieve chronic neuropathic pain, but its effect on prevention of PHN is unclear. Objective: To assess the efficacy of a 5-week course of gabapentin on acute herpetic pain and on the prevention of PHN at 12 weeks in patients with acute HZ. Methods Abstract Background: Varicella-zoster virus causes chickenpox and can reemerge later in life to cause herpes zoster or shingles. One of the most common and disabling complications of herpes zoster is postherpetic neuralgia (PHN). Conclusion: The results of this study show that gabapentin is effective in acute herpetic neuralgia in different doses with 600 mg/day being the more appropriate dose in terms of safety and efficacy. Diese betreffen vor allem Erwachsene und ältere Menschen, die an dem Varizella-Zoster-Virus erkranken. Frei verkäufliche Medikamente gegen eher leichte Schmerzen reichen in der Regel nicht aus, um in solchen Fällen Abhilfe zu Gabapentin is effective in reducing neuropathic pain due to post-herpetic neuralgia when given at least three times per day, due to its short half-life, resulting in demonstrable fluctuations in plasma levels. Gabapentin, given acutely, reduces the incidence of postherpetic neuralgia and might prevent episodes. The recognizable appearance and the dermatomal distribution of herpes zoster lesions usually enable a clinical diagnosis to be made easily. Postherpetic neuralgia (PHN) is the most common complication of herpes zoster (HZ). Previous trials have reported that gabapentin can relieve chronic neuropathic pain, but its effect on prevention of PHN is unclear. This randomized, double-blind, placebo-controlled crossover study measured the effect of a single dose of oral gabapentin (900 mg) on pain and allodynia associated with herpes zoster. Pain severity decreased by 66% with gabapentin compared to 33% with placebo.
